Read the denial reason line by line

Start with the explanation of benefits and denial code. Look for phrases such as not medically necessary, prior authorization missing, out of network, experimental, duplicate claim, or coding mismatch.

Each reason points to a different appeal packet. A medical necessity appeal needs clinical support; a coding dispute needs billing corrections.

Ask for the documents behind the decision

Request the plan language, clinical guideline, reviewer notes if available, and appeal instructions. Keep a dated record of the request.

Get your provider involved
