Quick answer

Optum Behavioral Health, the brand of United Behavioral Health, manages mental health and substance use benefits for many UnitedHealthcare plans. For commercial and Medicare Advantage claims, Optum uses a two-step process, reconsideration then appeal, with 12 months in total for both steps. Clinical appeals of authorization denials have 180 days. Optum names LOCUS, CALOCUS-CASII, ECSII and the ASAM Criteria.

Who manages behavioral health for UnitedHealthcare plans?

Optum does, operating as United Behavioral Health, a UnitedHealth Group subsidiary that has used the Optum brand since 2012. UnitedHealthcare's 2026 administrative guide says this affiliate delivers mental health and substance use benefits for commercial, Individual Exchange and Medicare Advantage plans, depending on the member's plan. Optum also serves some Medicaid plans.

The member's ID card and the denial letter show which entity decided. Optum runs employer EAPs under the name Emotional Wellbeing Solutions, and for some UnitedHealthcare Community Plan (Medicaid) members it provides the behavioral network and may manage the benefit, with state rules shaping the appeal.

UnitedHealthcare's national Gold Card exemption for eligible behavioral health services took effect on October 1, 2026 for providers that meet its approval-rate thresholds. For those providers an advance notification replaces prior authorization, so check your status before assuming an authorization was required.

02

What are Optum's appeal levels and deadlines?

For commercial and Medicare Advantage claims, Optum's 2026 network manual sets two steps: reconsideration first, then appeal, with 12 months in total for both unless state law or your agreement sets another deadline. Non-urgent clinical appeals of authorization denials have 180 days. The two-step process does not apply to Medicaid plans.

The two-step process covers claims denied or paid at a reduced rate, including for lack of prior authorization or lack of medical necessity, for dates of service from July 5, 2023. Optum decides on the materials in the file at the time of review, so send the treatment plan, clinical records and proof of timely filing with the first request. An overturn that means more money is reported on the remittance advice; an upheld decision comes by letter. After step 2, further review follows your participation agreement.

Clinical appeals of authorization decisions are filed by the member or an authorized representative, and the manual says the treating clinician at any level of care can be that representative. Filing details travel with the denial in the Member Rights enclosure. Federal rules sit underneath: employer plans must allow at least 180 days to appeal and must decide urgent appeals within 72 hours.

03

Should you file a reconsideration or a clinical appeal with Optum?

Match the route to the decision. If the patient is still in care and Optum denied or cut an authorization, follow the clinical appeal instructions in the adverse determination, and ask for expedited handling if delay is risky. If a finished claim was denied or underpaid, request reconsideration first, then appeal if Optum upholds it.

  • Concurrent-review denial, patient still admitted: clinical appeal, expedited if urgent. Start from the expedited continued-stay appeal.
  • Finished claim denied as not medically necessary ([CARC 50](/resources/codes/carc-50)): step 1 reconsideration with the records, then step 2 appeal if upheld.
  • Services given without authorization ([CARC 197](/resources/codes/carc-197)): check whether a retrospective review request still fits, within 180 days of the date of service.
  • Medicaid member: use the state process in the notice; see Medicaid managed care appeals.
04

How does peer-to-peer review work at Optum?

Optum's manual places peer review inside utilization management: outpatient cases that do not appear to meet criteria are referred to peer-to-peer review with an Optum peer reviewer, and for inpatient care Optum may require a conversation with the attending psychiatrist before authorizing. The manual publishes no fixed window for requesting one.

Care Advocates handle urgent determinations and appeals around the clock through the number on the member's ID card. Ask for the conversation as soon as you learn of the decision, record the reviewer's name and credentials, and do not let a pending call run past the appeal deadline. The peer-to-peer prep sheet keeps the call on the disputed criteria.

05

Which clinical criteria does Optum say it uses?

For mental health, Optum's 2026 manual names LOCUS for adults, CALOCUS-CASII for ages 6 to 18 and ECSII for ages 0 to 5. For substance use it uses the ASAM Criteria, and began implementing the fourth edition for commercial plans in November 2023. It also applies Optum Behavioral Clinical Policies and Medicare Coverage Summaries.

State law or the customer's contract can require different criteria. Facilities move to the ASAM fourth edition only after an updated contract is fully executed; until then third-edition rules apply. Ask which set and edition the reviewer used.

How do you get the criteria and the claim file from Optum?

Start with Optum's public Guidelines/Policies & Manuals page, which links the criteria and clinical policies. Then ask in writing for the specific criteria and edition used in your case, and for the claim file. Plans covering mental health and substance use benefits must disclose their medical-necessity criteria on request under the federal parity rules.

Optum's member-rights section says members are entitled to the reason for an adverse decision, including the specific criteria or benefit provisions used. For employer plans, the ERISA claims rule also gives claimants free copies of all documents relevant to the claim, including internal guidelines relied on. Optum's manual describes no separate provider claim-file request, so put the request in the reconsideration or appeal, or use the criteria request letter.

07

Where do you submit an Optum behavioral health appeal?

Use the Provider Express secure portal: choose Appeals, then Appeals Summary & Submission, then submit a claim appeal or a clinical appeal. The 2026 manual also lists a mailing address for processed claims. Optum's reconsideration form and quick reference guide, on the Provider Express forms page, carry the current instructions.

Older Optum forms list a fax line that the 2026 manual does not mention, so confirm the channel on the current form before relying on it. Keep the portal confirmation or mail tracking as proof of timely filing.

08

Which denial codes come up on Optum behavioral health claims?

The same claim adjustment reason codes appear on every payer's remittance. On behavioral health claims, the codes that decide the route include medical necessity (50), missing or exceeded authorization (197 and 198), denial at authorization (39), frequency or units (151), benefit limits (119) and provider eligibility (170 and 185).

Fictional example

A fictional Optum IOP claim dispute

Northgate Recovery (fictional) billed 12 IOP days for a UnitedHealthcare commercial member. Optum paid six and denied six as not medically necessary, citing the ASAM Criteria. The program's biller calendared the 12-month two-step window from the remittance date and noted the shorter deadline in its participation agreement.

Step 1 went through the Provider Express portal with the treatment plan, daily group notes and a two-page clinician letter that answered the dimension the reviewer scored lower. The letter asked which ASAM edition applied, since the program's contract had not yet moved to the fourth edition. Optum upheld three days and paid three; the program filed step 2 for the rest the same week.

Names, dates, payers and amounts are invented for illustration.

Where ClaimParrot fits

What a ClaimParrot packet does here

  • For a Optum Behavioral Health denial, an appeal packet drafts the letter for the appeal route this guide describes, and the deadline plan uses the date on your notice.
  • Each packet holds an appeal letter draft, an evidence checklist and a deadline plan, checked by a person before delivery: $29, or $99 for level-of-care denials, and the first packet is free (pricing).
  • You copy the payer's reason into the form with a de-identified summary; no notices, remittances or records are sent, and no PHI is accepted (Trust & security).

Every draft is reviewed and approved by your team. ClaimParrot does not make medical-necessity determinations or give legal advice.

Common questions

Frequently asked questions

Does Optum's 12-month window replace the member's 180-day appeal right?

No. They are different processes. The 12 months covers the provider's two-step claim dispute for commercial and Medicare Advantage claims. Member clinical appeals follow the Member Rights enclosure and the plan, and employer plans must allow at least 180 days. Work to the earliest deadline that applies.

Is Optum Behavioral Health the same company as UnitedHealthcare?

Both are part of UnitedHealth Group. Optum Behavioral Health, legally United Behavioral Health, administers behavioral benefits for many UnitedHealthcare plans, while UnitedHealthcare is the health plan. The denial letter shows which one decided, and the appeal goes where that letter says.

Does Optum still use its own level-of-care guidelines?

Its 2026 national manual lists LOCUS, CALOCUS-CASII, ECSII and the ASAM Criteria rather than a national Optum level-of-care guideline. State-specific guideline files still appear for some Medicaid programs on Provider Express, so ask which document the reviewer applied.

Can the treating clinician file a clinical appeal for an Optum member?

Yes. Optum's manual says the member's authorized representative may be the treating clinician at any level of care. Follow the Member Rights enclosure for the paperwork the plan needs, and keep the appeal on the elements the reviewer said were unmet.
